Mental Health Triage (1500_15)

I am emailing you to request information specifically concerning the mental health triage team in operation in the Wolverhampton area.

 

The information I want to access is as follows:

 

When working with a patient what information is recorded by the West Midlands police about that individual?

 

Since the introduction of the triage service how many patients has the team worked with? In addition I also would like the numbers to be broken down into the areas that those patients live in, in Wolverhampton

 

Since the introduction of the triage service, how many of the people the team have attended to work with already have an active referral to mental health services? specifically talking therapies.

When working with a patient what information is recorded by the West Midlands police about that individual?

 

Police record patients name, date of birth, address, contact telephone number, ethnic origin, gender, if in or have been in local authority care and if in or have served in armed forces.

Since the introduction of the triage service how many patients has the team worked with? In addition I also would like the numbers to be broken down into the areas that those patients live in, in Wolverhampton

 

Since the introduction of the triage service, how many of the people the team have attended to work with already have an active referral to mental health services? specifically talking therapies

 

Contact that triage have had with patients in Wolverhampton area to date total 491 patients of that there are 127 patients with active care plans.

We do not specify what active care plan a patient is engaged in, so active care plan could mean the patient is open to community mental health teams or has out-patients appointments, etc.

Our Triage record details in areas as local policing units (LPU) and do not further breakdown the smaller areas contained within each LPU and are unable to breakdown contacts with patients to areas contained within Wolverhampton LPU.
